If you have talked to him about it when you aren't already fighting and he hasn't changed his 'fighting' behavior then maybe you need to try some behavioral conditioning on him. Make sure there is a consequence (after the fight is over) if he calls you those names when you fight (e.g., withold sex for 1 day for each name, or don't cook for that many days, or you'll go stay with your parents for that many days, or something else he won't like). Once he knows about the consequences, then each time he calls you a name while you are fighting remind him by keeping the count out loud. You shouldn't have to do this very much before he gets the message.

tell him if it happens again you will leave him... and mean it. Otherwise it will never stop.
