How do I create a personal health record?

You start a personal health record by writing down all of the medical information you have about yourself. You also ask your doctor for copies of any lab or X-ray results of tests that you've had. If you've been hospitalized, get a copy of the medical record or at least get a copy of the discharge summary. If you are aware of any significant medical history in your family, such as heart disease, diabetes or cancer, include that in your medical record, and think of it as a diary of your health.
