I think part of the problem for doctors is; if they don't ask, patients won't tell. I talk about this a lot when I give lectures to physicians, they have to be open and available to be speaking about these issues. And if they're not comfortable about talking about it directly, then include some sort of sexual, health questionaire in their general health assessment, and then refer to that. In terms of a woman being embarrassed to talk about her sexual function issues, I'd say that your sexual health is a priority, you need to make it a priority, you need to address the issues. If he's not the person to talk to about it, then seek another health professional. Talk to the nurse and have the nurse refer that to the doctor. But you do need to get your issues across, and if you're dismissed, or if he's not emotionally or in any other way available to address those issues, then I tell patients, in this day and time to seek another health care provider.
