How do you register with a GP?

When you want to register with a new GP, it's actually very simple. All you do is you go along to the surgery and you fill in a form. You don't need to have your medical card, which most people have lost anyway. You just say to the receptionist, "I'd like to register at the practice." and that's all you need to do. You will have to fill in a standard form and most practices now will ask you to fill in a questionnaire which has lifestyle questions smoking, your diet, and exercise as well, which is data that GPs are now required to collect.
